Requesting Medical Records from Presbyterian/St. Luke's Medical Center

You can obtain a copy of your medical records from Presbyterian/St. Luke's Medical Center through our Health Information Management office. Please note that birth and death certificates are processed through the Colorado Department of Public Health and Environment.
