A decade ago, when the American Heart Association and the American College of Cardiology developed cardiac care guidelines – applying evidencebased theory to practice – P/SL became an early adopter and implemented them. We achieved the Gold Performance Level of the “ACTION Registry- Get with the Guidelines” (previously the American Heart Association’s GWTG-CAD program) in just one year, maintained it, and have been recognized again this year.
Cardiac Rehabilitation Manager Mary Jensen, RN-C, BSN, MBA, emphasized that in daily practice, “Get with the Guidelines” focuses resources so that patients receive the care they need while here, and return home with the education, medication and follow up plan.
Adhering to the Guidelines enables patients to be successful and avoid readmission that so frequently occurs with these conditions. “Obviously, to get this right 24 hours a day, seven days a week, requires that everyone makes sure patients have what is needed, and that every patient is treated the same way every time,” she said. “All of these therapies have proven to decrease their chance of another cardiac event and patients return home with the information they need to modify their risk factors.”
