Application Process for P/SL Pharmacy Residency
1. Our PGY1 residency application (upload as supplemental information)

2. A professional photo
3. A current curriculum vitae.
4. A letter of intent discussing what you hope to achieve by completing a pharmacy practice residency and the qualities you possess which you believe would help you be a successful resident.
5. Provide three letters of reference (two of which must be from pharmacists with whom you have had work or patient care experience). Letters of reference must be uploaded directly to PhORCAS.
6. Provide one copy of a current official transcript from pharmacy school.
